二、项目简表 表格使用说明: 请填写下面表格中所有的部分,全部内容(1―8)总字数不能超过1000字. 把这个项目简表连同报名申请表一起在2010年5月10日之前发送到gongyicishan@126.com.只接收电子申请. 注意:公司负责人要在项目简表上签字. 支持性的文件(印刷品资料或光盘),通过快递向组委会提交,请自付费. (1)奖项类别:说明项目申请的类别.奖项类别主要有:关爱健康、扶贫济困、环境保护、教育贡献、安全管理. (2)项目名称:提供一个申请的项目的名称(描述性的). (3)目标:简述项目预期达到的目标. (4)项目详细资料:解释一下为什么选这个项目,并详细描述.内容包括测算的数据、团队部门怎样开展工作、需要多长时间等等.也要说明参与人员的部门、职务(人员姓名不需要). (5)实施:项目是何时怎样改进实施的.(描述2008年或2009年实施或有明显增长趋势的项目). (6)结果:到目前为止有什么已经取得的定量和/或定性的成果,合作方、受助方反映怎么样,有什么具体的反馈意见. (7)其他信息:请提供其他与项目有关的你想提供的信息. 三、如何撰写申请,主要包括哪些内容 只有提交了项目简表,优秀企业公民示范单位奖的申请才会有效.The summary should not be more than

1000 words and it should state the name of the project or program, the category (or categories where the project is being entered) and a short descriptive title of the project or program.简表不应超过1000字,并应说明该项目或计划的名称,类别(或正要进入的类别)和对项目或计划名称的简短描述. It should also include the objectives of the project, describing what the company hoped to achieve and how it went about the implementation.它还应包括该项目的目标,描述一下公司希望实现什么,以及如何去实施的情况.Please indicate the results in measurements that were made in studying the problem or improvement being sought, or any studies conducted or reviews made.请说明研究有关问题或寻求改进的量化结果,或所做的研究或回顾. 它Please include how improvement was implemented, when and by whom.还应包括何时、由何人、如何改进实施.
