7, No.4 血管瘤是婴幼儿较常见的一种良性血管肿瘤, 发病率高达 10%~12%, 男女比例为 1:3~5[1] , 其病 因目前仍不十分清楚.近年来研究表明, 血管瘤是 一种血管形成性疾病, 血管内皮细胞的增殖和凋亡 在血管瘤的增生与退化过程中起重要作用.有研究 证实, 凋亡存在于婴幼儿血管瘤的整个自然过程[2] . Rb 和P21 是近年来发现的一种抑癌基因和细胞周 期素依赖蛋白激酶 ( CDK) 的负性调控因子. 能阻断 细胞周期从 G1 到S期转换, 从而抑制细胞增殖, 且 在某些肿瘤中存在基因异常和蛋白表达的异常, 能 促进肿瘤细胞凋亡.本研究应用免疫组织化学方法 检测不同时期婴幼儿皮肤血管瘤组织以及血管畸 形组织中 Rb 和P21 基因蛋白的表达, 探讨 Rb 和P21 在血管瘤发生、 发展以及退化过程中作用机制. 材料与方法

一、 实验材料与分组 本组

76 例, 血管瘤

54 例, 男34 例, 女42 例, 平均年龄 2.6 岁(40 d 至5岁) ;

血管畸形

22 例, 男12 例, 女10 例, 年龄 1~16 岁.按Mulliken[3] 分类标 准进行分组, 增生期血管瘤组 ( A 组)

28 例, 退化期 血管瘤组 ( B 组)

26 例, 血管畸形组 ( C 组)

2, 泸州医学院 附属医院小儿外科 ( 四川省, 646000) , E- mail: zouhuaxin607@163.com 【摘要】 目的 探讨 Rb 和P21 蛋白在婴幼儿血管瘤发生、 发展及退化过程中的表达及与血管瘤 退化有关的可能机制. 方法 采用免疫组织化学方法 ( SP 法) 检测 Rb、 P21WAF1/CIP1 及增殖细胞核抗原在 婴幼儿皮肤血管瘤标本增生期、 退化期以及血管畸形标本中的表达情况, 并用末端脱氧核苷酸基转移酶 介导的脱氧尿苷三磷酸 - 生物素颈端标记法检测血管内皮细胞凋亡情况, 结合 CD34 相关抗原的免疫组 织化学染色, 证实 Rb 和P21 表达的细胞是血管内皮细胞. 结果 增生期血管瘤内皮细胞 Rb 和P21 表达水平低于退化期, 差异有统计学意义 ( P <

0.05) , 退化期血管瘤 Rb 和P21 表达水平与血管畸形组织 相比, 差异有显著统计学意义 ( P <
